Mao ni ang mga requirements:a. Original copy and 1 photocopy of proof of enrollment:
Certificate of enrollment; orCertificate of registration; orSchool ID; orStatement of Account in school.b. 2 photocopies of Parents/Guardians’ Valid ID (if the beneficiary is a minor); or
C. 1 original or 1 photocopy of applicant’s valid ID if not a minor.

Kami sa LGU-Maasin City, thru CSWD Office, andam sa kanunay na mutabang or muassist sa mga programa sa national government para sa kaayuhan natong tanan.
Sa pagkakaron, gitagaan lang ang LGU-Maasin City ug directive sa pagdisseminate mahitungod aning educational assistance. Wala pay pormal na instructions from the national government na mu-assist sa karon na pay-out.
